                              Should you fail to verify any of Bank Negara Malaysia’s (“BNM”) Employee or Retiree or 
                             
      their Dependent through PMR or Mediline, please do the following:

                                     1. Verify that the Patient is a BNM’s Employee or Retiree or Dependent 
                                         by checking their Staff ID card or Kad Pesara.
                                     2. Fill up the PMCare GP Claim Form with Member's name, NRIC No. etc. and treatment charges. 
                                     3. Proceed with treatment.
                                     4. Submit the original GP Claim Form to PMCare.
